Designation Fee--------------------------$2000.00 Amount charged to use expert's name ,as applied
to the case.
Medical Record Review--------------------$575.00 per hour
Arbitration------------------------------$6900.00 for full day: Including travel time-$3450.00 for
half day--Half day Minimun requirement.
Deposition Fee---------------------------$720.00 Per hour for deposition, while waiting to give

Trial Testimony--------------------------$6900.00 For full day: including travel time-$3450.00
